Supportive services include development and implementation of family or individual action plan with a professional case manager who meets regularly with families or individuals to track progress and problem-solve when obstacles to action plan goals arise.
     
 

Each family receives case management within three days of entering the shelter. The case manager helps them to develop action plans which include full assessment of children’s developmental, educational, physical and mental health combined with analysis of family’s homelessness issues.

In the Mary Isaak Center once a person moves beyond the introductory phases of being at the shelter, they receive case management to develop an individual action plan. The case manager works with the client to track progress and help problem solve with obstacle arise.
